About VirtualER X-System

VirtualER X-System Version 7.0 is the proprietary operational and healthcare-connectivity platform that helps power the International Medical Direction (IMD) model. Developed by Virtual ER Network LLC, VirtualER works largely in the background to connect the people, information, workflows, physician oversight, education, quality systems, and operational tools required to support modern EMS medical direction.

A Different Model for EMS Medical Direction

International Medical Direction LLC was founded around a different vision for EMS medical direction: one in which independent, community-centered healthcare can extend beyond the traditional walls of the hospital and into the communities, workplaces, and homes where patients live.

Traditional healthcare delivery is often organized around bringing the patient to a healthcare facility. IMD believes EMS can play a much broader role. With appropriate physician medical direction, clinical governance, technology, education, and operational support, EMS can become an important extension of the healthcare system - helping connect patients with the right care, in the right setting, at the right time.

The Infrastructure Behind the Model

Making that model work requires more than traditional medical direction. It requires infrastructure capable of connecting physicians, EMS agencies, providers, healthcare facilities, education, quality assurance, clinical information, compliance, and operational workflows across otherwise separate organizations.

The VirtualER X-System is that infrastructure.

VirtualER is an evolving proprietary intellectual-property and operational platform designed to help IMD transform traditional EMS medical direction into an integrated framework for community-centered care. It provides a common technology foundation from which medical-direction programs, community healthcare initiatives, emergency-response operations, and supporting administrative workflows can be developed and scaled.

System Focus

VirtualER is being designed around the convergence of EMS medicine, emergency response, healthcare interoperability, education, quality assurance, dispatch, GIS, compliance, and outcome analysis. Rather than replacing the systems that create operational and clinical data, VirtualER is intended to connect them through a normalized internal model and versioned interoperability layer so authorized users can understand the complete operational and clinical picture.

The long-term objective is to support meaningful exchange and reporting across NEMSIS EMS data, NERIS fire and emergency-response data, HL7/FHIR healthcare information, CAD/RMS systems, hospitals and health-information exchanges, GIS services, registries, learning-management systems, and other approved data sources.

Connected Data & Outcomes

VirtualER uses a canonical relational data model so information from different standards can intersect without forcing one external schema to become the system itself. Versioned adapters and terminology mappings are intended to translate inbound and outbound data while preserving source identifiers, provenance, validation, and audit history.

This architecture can connect the progression from dispatch and incident response through EMS care, transport, receiving-facility information, physician medical direction, quality review, education, compliance, and clinical outcomes. Authorized reporting can therefore examine not only what occurred in the field, but how operational decisions, treatments, destinations, and system performance relate to later outcomes.

GIS, Disasters & Large-Scale Events

GIS and event-location data are planned as shared system capabilities rather than isolated map features. Future VirtualER child applications may use the same canonical incidents, people, patients, organizations, resources, facilities, and locations to support disasters, mass-casualty incidents, evacuations, wildfire and austere deployments, patient movement, destination tracking, resource positioning, alternate-care sites, and regional situational awareness.

Disaster and large-event workflows should extend the same identities and interoperability contracts used during routine operations. This avoids creating separate disconnected populations of records and allows authorized information to follow an incident or patient across operational contexts while preserving privacy, security, provenance, and source-system ownership.

Version 7.0 Foundation

Version 7.0 is a full redevelopment of the VirtualER X-System platform with a normalized PostgreSQL data model, profile-, scope-, role-, and access-based authorization, mandatory multi-factor authentication, audit and security event architecture, responsive navigation, independently scalable application modules, and a stateless application design intended for externally managed data and service infrastructure.

Current Foundation

Security & IdentityControlled onboarding, email verification, mandatory 2FA, canonical Person identity, scoped profiles, server-side authorization, and structured Audit, Security, Access, and Activity event domains.
Data ArchitectureRelational-first PostgreSQL design, normalized operational entities, controlled reference data, external durable storage boundaries, and stateless application compute.
Interoperability FoundationCanonical domain modeling with versioned mappings and adapters for NEMSIS, NERIS, HL7/FHIR, CAD/RMS, GIS, education platforms, and future approved systems.
Application FamilyMain is the hub and reference implementation for future independently hosted VirtualER applications that share security, data, UI, and interoperability contracts.
